A Licensed Clinical Social Worker, Tara has spent her 25-year career working with families in a variety of clinical settings. Her passion has always been to strengthen and support families through challenges. She has done extensive work in trauma, abuse, attachment, loss and grief. Tara currently serves as the Associate Executive Director of Culture and Experience for Peconic Bay Medical Center-Northwell Health.
Tara received her Master of Social Work degree from Fordham University. She is a Doctoral Candidate, at Molloy University where her research is focused on family caregiving. She is proud to serve on Molloy University’s Annual Palliative Care Planning Committee, Northwell Health’s Advanced Illness Faculty, and the Long Island Alzheimer’s Association’s Annual Caregiver Conference Planning Committee.
Tara joined Peconic Bay Medical Center-Northwell Health in 2013, where she began on the Palliative Medicine team. The team implemented community MOLST education across eastern Suffolk County, established the hospital’s family caregiver support program, created a bereavement plan of care for individuals who experience loss, and led the hospital’s inaugural Joint Commission Palliative Care Certification in 201
In 2018, she established the first hospital-based Caregivers Center on Long Island dedicated to providing person centered supports, education and resources to family caregivers. The Caregivers Center led by a social worker, has a unique model which utilizes trained volunteer caregiver coaches to provide peer support to family caregivers. This program has been supported more than 5000 family caregivers since it opened.

Carol Markman
Carol’s journey as a caregiver for her husband Bob began after he was diagnosed with Mild Cognitive Impairment in 2014. Although looking back there were signs of cognitive decline earlier. Bob started to attend the programming at the LIAD Center in October 2018 and loved the time he spent at the Center. The pandemic created challenges for everyone and LIAD Center came through with on-line programing that Bob really enjoyed and benefited from.
Carol is an active participant in the Thursday caregiver group and is so grateful for the help and loving support the group provides to her. She looks forward to giving back to others the support she received.
Carol accepted this honor before Bob passed away in December 2022 and is humbled to do so. She has a daughter, June, whom she lives with and her husband, Ken, in Connecticut and a son Charles who lives with his wife and grandchildren in Michigan. Carol is very grateful for all the support and caring she has received from her friends, community and LIADC.

OUR MISSION
Our mission is to help improve the quality of life for those living with Alzheimer’s Disease and other related dementias, and their caregivers. For over 30 years the Long Island Alzheimer’s & Dementia (LIAD) Center has served as a lifeline to diagnosed individuals and their families throughout Long Island. The LIAD Center takes considerable pride in its pioneering role in the development of cutting-edge services that foster the independence, dignity well-being and safety of individuals with dementia, thus aiding in the prevention of premature nursing home placement. We are committed to the belief that there is life worth living after a diagnosis. Our caring professionals help program participants maximize their remaining abilities, while providing much-needed information, support and respite to family caregivers.
